Team Approach Policies

The following policies apply for our therapeutic massage and physical therapy services.

Available hours: By appointment only

Forms of Payment: Payment accepted as cash or checks. Sorry, no credit cards. There is a $25 fee for any check returned for insufficient funds or closed accounts. Payment is due for services rendered at the time of treatment.

Insurance Claims: Insurance forms are not handled in this office. However, we will provide you with a receipt which you may submit to your carrier for possible reimbursement if you wish.

Missed and late appointments: As a courtesy to other clients, if you are late, session time may be curtailed. Payment of 59% of session fee is expected if you fail to cancel, or cancel in less than 24 hours. All efforts will be made to reschedule you as quickly as possible.

Massage packages: All packages are pre-paid in full. All packages must be used within 1 year of purchase date.
